Using insurance
We bill your plan directly. You pay your copay, coinsurance or deductible amount, exactly as you would for an in-person visit.
HSA and FSA
Both are accepted for self-pay visits, and we provide an itemised receipt you can submit for reimbursement.
What is not included
Labs, imaging and medication are billed by the lab, imaging centre or pharmacy, not by us. We tell you when an order carries a cost.

What does a visit cost without insurance?+
Self-pay prices are posted: from $100 initial and $75 follow-up for primary care, $200 and $150 for mental health, $150 and $100 for weight loss and men's or women's health, $100 and $50 for pediatrics, and $50 for refills and accommodation letters.
Which insurance plans do you accept?+
Medicaid of Indiana, Hoosier Healthwise, the Healthy Indiana Plan, Medicare, UnitedHealthcare, Anthem Blue Cross Blue Shield, Aetna, Humana, Cigna and TRICARE, among others.
Can I use an HSA or FSA card?+
Yes. HSA and FSA cards are accepted for self-pay visits, and we provide an itemised receipt.
Will I get a surprise bill?+
No. Self-pay prices are posted in advance and charged at booking. If you use insurance, your plan's copay or deductible applies and we tell you what we know before the visit.
